In collaboration with U.S. Department of Health and Human Services (HHS), the Centers for Disease Control and Prevention (CDC), state and federally funded programs, you will implement a project focused on reducing chronic disease rates within a defined geographic area within four communities: Lake County, Indiana; Sandusky and Toledo metro area, Ohio; Brownsville, Texas; Petersburg, Virginia.
