The goal of this funding opportunity is to award a cooperative agreement to up to five entities to develop and implement multi-faceted strategies that support the delivery and sustainability of evidence-based chronic disease self-management education (CDSME) programs for older adults with one or more behavioral health conditions.

Additional information

Faith-based and community organizations that meet the eligibility requirements are eligible to receive awards under this funding opportunity announcement. Foreign entities are not eligible to compete for, or receive, awards made under this announcement.
