Description

Cooperative agreements awarded under the Alzheimer's Disease Programs Initiative (ADPI) State and Territory Programs for Dementia Specific Respite  Notice of Funding Opportunity (NOFO) 36 month program are intended to expand and strengthen supports for unpaid family caregivers by increasing access to high-quality, dementia-specific respite services delivered by dementia-trained providers through the National Aging Services Network, including State Units on Aging (SUAs), Area Agencies on Aging (AAAs), and aging network service providers. The program will support unpaid family caregivers of people living with a probable diagnosis of Alzheimer's disease or related dementias (ADRD) who need access or increased access to respite care and related services. The primary goal is to reduce caregiver burden and support delayed institutionalization by increasing the availability and delivery of dementia-specific respite services that are responsive to the unique needs, preferences, and circumstances of people living with ADRD and their caregivers.
Through this demonstration grant opportunity, successful SUA applicants will partner with AAAs and other appropriate National Aging Services Network providers to coordinate and deliver dementia-specific respite services comparable to those provided under Title III-E, the National Family Caregiver Support Program of the Older Americans Act (OAA). Respite services will be delivered by providers with appropriate dementia-specific training, knowledge, and skills to support the needs, preferences, well-being, and safety of people living with a probable diagnosis of ADRD. Grant funds will be awarded to successful SUA applicants to establish a new dementia-specific respite program or expand an existing program. SUAs will distribute funds to appropriate National Aging Services Network providers to support program implementation, including provider training and preparedness and the delivery of high-quality, dementia-capable respite services.
Dementia-specific respite services will be made available to eligible unpaid family caregivers of people living with a probable diagnosis of ADRD, including caregivers of people under age 60 with a younger-onset dementia diagnosis who otherwise meet the eligibility requirements of the OAA Title III-E National Family Caregiver Support Program. Successful SUA applicants will establish the terms and conditions governing implementation of their dementia-specific respite programs, including requirements to ensure that respite providers are appropriately trained and prepared to deliver strengths-based, person- and family-centered services that are responsive to the needs of people living with ADRD and their caregivers. Successful applicants will also conduct an independent, third-party evaluation to assess and document the program's impact on participating unpaid family caregivers and community-residing people living with a probable diagnosis of ADRD.
